1. Membership Term
The term of this membership shall commence on [date of this form] and shall continue for twelve (12) months consecutively. After 12 month term, monthly fees will continue to be charged on a month to month basis, unless terminated in accordance with this Agreement.
2. Membership Fees
Member agrees to pay a monthly membership fee of one of the following options:
$80.00 / month for Elite Class Tanning or
$70.00 / month for Gold Tier Tanning
Payments will be processed on the same day of each month. [Commencing on the date of this form]
A one time set-up fee of $20.00 will be charged to the first payment only
3. Cancellation Policy
After the initial twelve (12) month commitment, the membership may be canceled with thirty (30) days' written notice to the Organization, pro-rated to the next monthly payment at the date of termination.
4. Membership Perks
Member shall receive the following benefits:
Unlimited UV Tanning offered by Bossy Glam Studio Inc
Elite Class offers access to all UV Tanning Beds/Stand-up Booths, Gold Tier offers access to ONLY Gold tier UV tanning equipment
10% off all UV tanning retail products
5. Responsibilities of Member
Member agrees to:
Abide by the policies and procedures set forth by Bossy Glam Studio Inc.
Maintain respectful and professional conduct while utilizing services and interacting with staff.
NSF Fees & Non-Payment of Membership dues
2 day grace period is in effect for all failed payments, before attempting to re-collect payment. Member is responsible for any NSF fees associated to the failed/re-try attempts to collect payment.
If more than 2 consecutive payments are missed/failed - membership will be revoked and member will be financially liable for the remaining months in the agreement.
6. Liability
Bossy Glam Studios Inc. shall not be liable for any damages or injuries incurred by the Member during the membership term.
7. Governing Law
This Agreement shall be governed by the laws of the Province of British Columbia, Canada
8. Entire Agreement
This Agreement constitutes the entire understanding between the parties and supersedes all prior agreements, oral or written, related to the subject matter herein.
